Los Angeles Appellate Forms and Court Resources

California appellate proceedings use a combination of statewide Judicial Council forms and court-specific resources maintained by Los Angeles Superior Court and the Second Appellate District.

The correct form depends upon the case type, reviewing court, and procedural step.

Start With the Correct Pathway

Before selecting a form, determine whether the matter proceeds to the Second Appellate District or to the Los Angeles Superior Court Appellate Division.

Statewide Judicial Council Forms

The Judicial Council maintains separate appellate form families for unlimited civil, limited civil, felony, misdemeanor, infraction, and other appellate proceedings.

Common unlimited-civil forms include the notice of appeal, record designation, civil case information statement, extension, dismissal, certificate of interested entities, and proof-of-service forms.

The current Judicial Council Forms Library should be used rather than an independently stored copy because form versions can change without the basic form number changing.

Second District Local Forms

The Second District maintains local forms in addition to statewide forms. Its current library includes forms for record designation, extensions, augmentation-related proposed orders, proof of service, record retrieval, and dismissal-related requests.

Use Current Forms and Rules

Before filing, confirm the current form, current revision/effective date, issuing body, any local alternative, and the electronic-filing requirements that apply to that filing.

Court forms and procedures can change even when a bookmarked URL continues to work.
